As with most things, it depends. It is definitely not required. The first criterion is whether it's good or not. For resume/eye candy purposes that threshold is 3.5 (assuming a 4.0 scale - adjust accordingly for whatever scale your school uses). If you're below that, don't include it.
The second criterion is where you are in your career. If you're a college senior applying for your first full-time role for post-college, and your GPA is 3.5+, DEFINITELY include it! It shows you are competent, consistent, and cared enough over the last four years to do well. If you're still in school applying for internships, or are a high school student, then it's fair game to include it as well.
I would say the cutoff for including it is 2 years out. So, if you've worked your first job post-college for two years, and now you're applying for something new, your GPA no longer belongs on your resume. Resume readers are going to care a lot more about your impact and achievements in the professional world over the last couple of years than about how well you did in college.
Same rules apply for graduate level degrees (MBA, Master's, etc). If you're still finishing up or looking for your first position post-degree, and your GPA is "good", put it on there. If you're 2+ years past the degree, take it off.
While not specific to GPA, if you were so blessed and/or dedicated to graduate cum laude, summa cum laude, or magna cum laude, keep that shit on your resume forever! It falls more into the notable honors/achievements category and hits different than some 30-something year old nerd saying they got a 3.9.
...for the record, I (Nate) got a 3.3 in college including one F and it never affected my professional career at all. Once you're in the working world, your impact there is what actually matters!
